Fix slab name "biovec-(1<<(21-12))"

The filename of the patch is: fix-slab-name-biovec-1-21-12.patch and it can be found in the queue-4.14 subdirectory.

If you, or anyone else, feels it should not be added to the stable tree, please let stable@vger.kernel.org know about it.

I'm getting a slab named "biovec-(1<<(21-12))". It is caused by unintended expansion of the macro BIO_MAX_PAGES. This patch renames it to biovec-max.
